North Star Manor is in Warren, a small town in the scenic northwest corner of Minnesota. The home shares a campus with North Valley Health Center, a critical access hospital. Warren is near I-29 and about 30 miles from Grand Forks, North Dakota, where there is a large airport.
North Star Manor offers three levels of care: independent living, assisted living and long-term care. The assisted living and long-term care units are attached to the hospital, making access easy during even the worst weather. Assisted living residents receive periodic oversight from a registered nurse, and a nurse is always on call. Long-term care residents can benefit from a range of therapies. All long-term staff are trained to help those with Alzheimer���s.

Independent and assisted living residents can choose from a variety of one- and two-bedroom floor plans. In the assisted living wing, North Star provides light housekeeping, laundry services and three meals daily. The facility provides a varied schedule of group activities for long-term care residents. Religious services are available for all three units.
