Welcome to Elm Wood Center! Elm Wood Center, Skilled Nursing and Rehabilitation, provides both LongTerm Care and ShortStay Rehabilitation. We offer Skilled Nursing, Palliative Care, Hospice Care, Respite Stays, Medical and Rehabilitative Care for patients and residents. Elm Wood Center provides daily therapy services including Speech Therapy, Occupational Therapy and Physical Therapy along with 24-hour nursing care. Elm Wood has a spacious dining room offering restaurant-style dining and patients and residents are offered a choice for meals, including an always available menu.
