When a client begins service with Assisting Hands®, a full assessment of needs will be done by our Director of Nursing and a care plan will be implemented to address each of the needs identified.

It is our philosophy during the client assessment to also understand the personality of the person and the type of caregiver that would make the best fit for that individual. We take the time to review the personality of each client and then review available caregivers that make the best fit. We then assign that caregiver to the client and keep that caregiver with the client on an ongoing basis. We believe that the best care is provided when trust is established and when the caregiver really gets to know the client. So unless the client or caregiver requests a change, it’s our policy to keep the caregiver assigned to ‘their’ client.

It is also the policy of Assisting Hands Home Care to effectively coordinate our services with any person or agency providing care to our clients. We communicate with the client’s primary care physician and specialists on a regular basis on the status of our joint client. If there is a change in that status, we communicate that immediately. We understand that we are part of our client’s continuum of care and we share progress with all those involved.
