PHC is a non-technical, medically related personal care service prescribed by the physician as part of a client’s plan of care. PHC is available to eligible Medicaid clients whose health problems cause them to be functionally limited in performing activities of daily living. A PHC attendant provides each client’s needed services.
Personal Care that includes bathing; dressing; preparing meals; feeding; exercising; grooming; caring for routine hair and skin needs; taking self –administered medications; toileting, and transferring and ambulating
Home Management Services include assistance with housekeeping activities that support the client’s health and safety. These activities include: changing bed linens; housecleaning; laundering; shopping; storing purchased items; and washing dishes
Escort. Accompanying the client on trips to obtain medical diagnosis or treatment or both. This service does not include direct transportation of the client by the attendant.
