Welcome to Four Seasons Community Care! Four Seasons Care Home, Inc. is a licensed level III residential care facility located in the picturesque village of Northfield, Vermont.
Four Seasons is a 37 bed care facility who caters to the needs of adults of any age. We participate in the ACCS program (Assisted Community Care Services) and are approved to provide Choices For Care, a component of the Medicaid waiver program. The beauty of this program is that it allows your loved one to age in place. This means they would likely not need to transition to a nursing home when they reach a new level of increased care.
Four Seasons offers a warm and welcoming environment that both supports and encourages the residents physically and emotionally. Offering and engaging our folks in a variety of activities helps to emphasize healthy aging and quality of life. Safe surroundings promote independence and enhance respect and dignity.
Our facility is located in the heart of the Vermont. Our community is one of integrity and kind-heartedness. Northfield is also the home of the oldest private military college in the United States, Norwich University. The facility is within walking distance to the local clinic, restaurants, banks, post office, local stores and the Senior Center.
