Our Executive Director has been serving older adults their entire career. They bring a wealth of passion, knowledge and expertise to serving our older adults. We have a nurse practitioner and physician from the Kansas City Medical Partners on staff as well.
2.Physical, Occupational and Speech Therapy. Autumn Leaves has 3rd party vendor agreements with exceptional providers of PT/OT/ST. The same therapists come to our communities to provide on-going therapy support as ordered and directed by our residents physicians.
3.Person-Centered Care. We believe in purposeful living that honors individuality, respects personal dignity, and preserves family legacies. We achieve this by customizing every aspect of care to your loved one’s life story: interests, hobbies, and career – as well as likes, dislikes, wants, needs, and aspirations. Our Life Engagement Manager, is a ray of sunshine and always has something fun going on, whether it is a group or individually personalized activity.
4.Dementia-Trained Staff. Employees are trained in dementia care best practices using educational tools and programs developed by the Alzheimer’s Association, the National Council of Certified Dementia Practitioners, and other educational resources.
5.Respite and Day-Stay Options. Being a caregiver is hard. It is taxing mentally, physically and emotionally. We are here to care for your loved one so you can care for yourself.
43 Residents are able to live in their home with us through the end of life. Just like we have home health vendors, we have a hospice vendors who come in when palliative or hospice care is needed.
