San Simeon by the Sound

San Simeon Adult Day Health Care can meet the needs of 30 Registrants, Monday-Friday from 9:00am-4:30pm. We care for those with a wide variety of medical conditions associated with aging – chronic illnesses, such as arthritis, heart conditions, high blood pressure, diabetes and cognitive deficits, as well as those with Alzheimer’s disease. Through a single point of entry, clients and their families will gain access to coordinated caregiver support, healthcare, rehabilitative services and respite care.

Our program is the only one of it’s kind on the East End of Long Island to: Provide the maximum level of independence; Restore and rehabilitate to the highest level of functionality and prevent or delay further deterioration as long as possible; Provide support, respite and education for families and other home caregivers; Foster socialization and peer interaction; Serve as an integral part of the community network and long-term care continuum for every patient. Services include: transportation, morning and afternoon snacks and mid-day meal, recreational activities, and onsite Audiology, Cardiology, Dental Care, Podiatry, and Orthopedic services.
