Housed in a small white building, Valley Pointe shares a neighborhood with apartments and office buildings. Although the facility accepts patients with mild dementia, the facility is not equipped with Wanderguard and there is no specific activity programming for individuals with dementia.

The carpeted living room is small but cozy. Pretty upholstered chairs, a comfortable couch and an aquarium made us forget, for a moment, that we were in a SNF! Guests gather here, as there are no other seating areas in the facility. The rest of the interior is well-maintained but retains an institutional feel. Windows allow sunlight to stream into the main dining room, which is also used for activities. Residents who require feeding assistance take their meals in a smaller dining room. The shaded patio alongside the building features freshly mown grass, planted trees and an appealing seating area. Residents enjoy barbecues on the patio during the warmer months; the area is also popular with smokers. The carpeted hallways, which have handrails installed on both sides, are brightened by fluorescent lighting. Many residents sat in their wheelchairs, filling the halls. …. SHOW MORE

Valley Pointe offers private rooms in addition to two, three and four-bed rooms. The moderately sized rooms are furnished with nightstands, two small drawers and one closet per person. All rooms have windows with flowered drapes and linoleum floors. The bathrooms are shared.

We spoke with several staff members, whose tenure ranged between three and ten years. Our guide proudly detailed a special program for the short-term rehab patients, Homeward Bound, in which the goal is to rehab patients so that they may return home. It begins with a hospital visit and coordination of a treatment plan. Once the patient is transferred to Valley Pointe, the therapy begins. Participants reside in a separate house at back of the facility, which features a kitchen, dining room and bedrooms. Here, the patient learns the skills needed to return home; for example, getting in and out of bed, cooking and carrying groceries. The program also includes a home visit before the patient returns, to determine, more specifically, the patient’s home needs.
