Medication Set-Up and Medication changes: Medications will be set-up by a nurse every other week per Dr. orders. Medication changes will be made per physician orders for an additional nursing charge.

Medication Reminders or Administration: The Resident Assistant will provide vocal reminders or assistance with oral medications, PRN medications, eye medications, eardrops, inhaled medications, topical medications, and rectal medications (insulin not included). All medications we assist with must be ordered by a Doctor. If it is determined by the RN assessment that medications cannot safely be stored in the apartment unlocked, a lock box will be provided at the client/family expense.

Insulin Set-Up: Insulin will be set up weekly by the licensed nurse per Dr. orders. Insulin changes will be made per Dr. orders for an additional nursing charge.

General Assessment: Visit by a nurse to include Vital Signs and basic head to toe assessment.

Nursing Services: Incidental nursing services related to but not limited to falls, illness, injury, or infection. Incidental nursing services at client request will always incur an additional nursing charge.

Blood Sugar Checks: Resident Assistant can assist with Blood Sugar checks per doctor order. Resident assistant will be trained to identify symptoms of a diabetic reaction and will have orders to obtain an additional blood sugar reading if a reaction is suspected.

Personal Cares: The Resident Assistant arrives at the client’s apartment to assist with: dressing, oral cares, skin care, hair care, peri-care, TED hose application/removal as prescribed, and AM/PM cares (dressing and grooming services needed to prepare for the day and/or for bed.) Each client is encouraged to assist as much as they can to maintain their highest level of function.

Bathing Assistance: Bathing assistance will vary according to the individual client’s needs. Assistance can range from hands-on assist to stand by assistance. This service is provided within the client’s own apartment setting by the resident assistant. Hair shampooing and nail care will be included at this time on an as needed or as requested basis.

Vital Sign Monitoring: The Resident Assistant will obtain Vital Signs which may include Blood Pressure, Pulse, Respirations, Weight, or Temperature. This service includes obtaining, recording and reporting as needed.
Laundry: Laundry service is available at a rate of $6/load which includes pick up and delivery to the client’s apartment. Resident Assistants can change linens on 1 bed/apartment/week per your request or on a scheduled basis at the rate of $6.00.

Meal Delivery: Meal deliveries are scheduled deliveries of the lunch meal for client’s who do not wish to attend the community dining room.

Housekeeping: Weekly light housekeeping in the kitchen and bathroom is included in the rent of all apartments. Additional cleaning of the apartment can be added at the Resident Assistant rate. This may include items such as vacuuming or dusting.

Miscellaneous Services: Assistance of one staff person to transfer, ambulate, or assist to toilet is available by our on-site staff. If additional staff is needed, other arrangements with family or outside providers will be made. Other services ordered by a physician or deemed necessary by a medical professional may be available at the above listed Ala Carte rates based on the discipline providing the service. Examples of these services may include: ace wraps, minor dressing changes, PT ordered exercises, therapeutic ambulation, or Range of Motion (ROM) for maintenance purposes.

Nail Care: Care of fingernails and/or toenails by a nurse can be selected on a weekly basis. Service includes foot soak and lotion application.

Oxygen Management: The Professional nurse will assist with oxygen management by communication with the oxygen vendor and oximetry. The Resident Assistant will be trained in oxygen management including filling portable oxygen systems and monitoring the appropriate liter flow as indicated by the physician order.

Other Information:

• If a client is requiring immediate emergency treatment, the first response is to dial 911. Aides will respond to the call pendant as soon as possible.

• The client or responsible party must provide supplies required for the delivery of care. The cost of the supplies is not included in the cost of the health care services.
