The Oaks is an Assisted Living Community with a caring, safe and secure, home-like environment for its residents. Unlike a nursing home, The Oaks provides supervision in an elegant setting with the highest level of care for its seniors who are either still independent or require differing levels of care. The Oaks is staffed with professionally trained caregivers, selected for their knowledge, compassion and understanding of our residents needs. Each apartment is carefully designed to encourage independent living, while providing 24-hour care and assistance with daily living activities. We are proud to be a part of the community of Forsyth County. This area was chosen for its quiet and relaxed atmosphere and for its close proximity to churches, shopping and medical facilities. Adult Daycare Adult Daycare provides a unique and ideal situation for those who are not prepared to change their current residence, but need daily supervision, activities and social interaction. Guests will be involved with the daily activities and with it residents. Adult Daycare is offered between the hours of 7:00 a.m. and 7:00 p.m. We do not recommend that the guest stay longer than eight hours. One meal and a snack are provided. Additional meals, beauty salon/barber shop, podiatrist and arrangements for medical appointments are charged separately as stated on the Schedule of Fees. Admissions information is available upon request. Respite Care Respite Care is provided for guests who require short-term stay with 24-hour supervision. The Oaks offers this service for a daily fee, provided there are apartments available. Advance reservations are required. Respite Care allows family caregivers time off for vacations, and, in some cases of emergency, gives them peace of mind that the adult is cared for properly. Guests will enjoy all the amenities offered at The Oaks. We are genuinely excited about this service and feel it will benefit many in our community.

Our Home With a bright and open country setting, our dining and family rooms minimize agitation and confusion. Our design is simple with easy to find bedrooms and gardens so that are residents are at ease, secure and have freedom to enjoy their lives. Our Family We encourage family participation in holiday and special celebrations. The best part of our home is the family atmosphere that is created when our families are involved together in the everyday lives of our loved ones. The Salabarria family has always believed in family harmony and togetherness and value the importance it plays in our facility. Our Daily Living Our daily activities begin every morning upon waking with our residents preparing the country dining area for breakfast, baking fresh bread and participating in all aspects of daily living activities. Specially designed memory and life enrichment areas take our residents back in time to the days they hold and cherish the most. Our Community Director leads programs that incorporates our residents history, preferences and abilities into individualized care plans. Our Staff For Horizons, we look for the most important skills from our applicants. Those skills being love, compassion and the sincere adoration of our residents. Their credentials are gained through training specific to memory loss and Alzheimer's.

Levels of Care
BASIC LEVEL Provides all the comforts of home and encourages independent living. • Three well-balanced, delicious meals and two snacks daily • Specialized diets • 24-hour staff available thru emergency call system with 2-way communication • Monthly scheduled activities and social events • Scheduled transportation to shopping, dining and special events • Monthly health monitoring • General monitoring of residents functioning and condition level • Scheduled transportation to medical appointments • Daily spot cleaning and trash removal from apartment • Weekly detailed room cleaning • Linens and towels provided and changed weekly LEVEL TWO
Provides all the Basic Level services, plus: • Individual nutritional meals dealing with mechanically-altered diets • Frequent support with ambulation • Prompting in areas of dressing, grooming and other hygiene LEVEL THREE Provides the Basic Level and Level Two services, plus: • Assistance with dressing, grooming, toileting and bathing • Assistance with transfers • Assistance with eating, including cutting and use of condiments • Close monitoring of physical whereabouts and condition with 24-hour periodic resident checks • Personal laundry service LEVEL FOUR
Provides all of our levels of service and the following for residents needing a high level of care: • Assistance with eating, including complete nutritional supervision • Assistance with clean-up of chronic incontinence • Close monitoring to ensure that our services and facility are appropriate for residents' health needs
