The objective is to meet the physical & psychological needs of an individual in time of illness, disability, or emotional crisis, which may threaten the normal pattern of life. We provide coordination of services for residents that are eligible to receive home health services under physician’s orders & Medicare criteria. Services include: Home Health Aides for companionship, personal care and assistance with the activities of daily living, Nursing Services including Wound Care, Diabetic Care and Blood Pressure Monitoring, Rehabilitation Services including strengthening, gait training, home safety & exercises.
