Assisted living at the Good Samaritan Society - Mesa Good Shepherd encourages
residents to continue living a full lifestyle while receiving services designed
for each resident’s unique needs. Residents enjoy three home-cooked meals per
day, as well as an array of cultural, spiritual, educational and social
activities. Residents also enjoy shopping, dinning, movies, field trips and
monthly special events.
Our registered nurse and care professionals are available 24 hours a day to
provide residents with an extra level of care, including personalized medication
management, showering and bathing assistance, dressing and grooming assistance,
housekeeping and laundry services and appointment escorts, while promoting
continued well-being through health and wellness programs. Transportation is
available to take residents to their own healthcare providers and activities.
Several floor plans enable you to choose an apartment home that fits your
lifestyle. Whether you chose a studio or one-bedroom deluxe apartment, our
single-level architecture provides easy access to all residential and community
areas.

Skilled Nursing

Health Care and Rehabilitation Center
Mesa Good Shepherd's Health Care and Rehabilitation Center is a Medicare and
ALTCS approved 80-bed skilled nursing center designed to meet your short- or
long-term care needs by providing skilled and compassionate care. Our excellent
staff and full range of services are available to assist in meeting your care
needs.
The center's medical director, licensed nurses and certified nursing assistants
provide skilled nursing and assistance 24 hours a day. To better meet
individualized needs, the center's Care Plan Team, consisting of staff,
residents, and family members participate in the development of personalized
care for you or your loved one. Mesa Good Shepherd also offers a rehabilitation
department that provides speech, occupational and physical therapy to meet
residents’ rehabilitation goals.
You can count on diversified resident-centered activities and conveniences. Our
social services director provides resident advocate services, and additional
health services such as podiatry, psychiatry, optometry and dental services are
arranged for our residents. Mesa Good Shepherd also offers a beauty/barber shop
and a beautiful chapel. Our chaplain is an active part of our resident care team

Therapy Rehab
No one plans to be sick or have to recover from a serious injury, but it's nice
to know if you have to that you have options. Although your first stop is the
hospital often you may require rehab therapy. Whether it's outpatient,
inpatient, short term or a bit longer now you can choose a program with a
homelike atmosphere. Everyone need a little bit of help sometimes. With round
the clock care and a therapy program that is structured to meet your recovery
goals our staff is here to assist YOU. So whether its a broken hip, or you are
recovering from a stroke our team of professionals is focused on your goals
because YOU are the most important member of YOUR recovery.

Medicare Information
Medicare is the federal government's health insurance program for people 65
years of age and older. It also covers many people who are disabled.
Two Parts of Medicare:
Part A: Also known as "hospital insurance," it is financed through Social
Security (FICA) payroll contributions paid by employees, employers, and
self-employed persons.
Part B: Also known as "medical insurance," it is financed by monthly premiums of
people who voluntarily enroll in the program. The monthly premium is deducted
from their social security, railroad retirement, or civil service check.
Medicare Part A provides coverage for services in a skilled nursing facility (SNF)
under certain conditions. The following criteria must be met to qualify for Part
A benefits.
The resident must:
1. Be hospitalized at an acute level of care for three consecutive days (not
counting the day of discharge before entering the skilled nursing facility).
2. Need the services of the SNF on a daily basis.
3. Be admitted to the SNF within 30 days after discharge from the hospital.
4. Have the services ordered by the physician.
5. Have benefit days left in the benefit period.

The Good Samaritan Society - Mesa Good Shepherd's Senior Living Apartments offer
you the best of apartment living with the fellowship of a warm, caring
community. The Good Samaritan Society - Mesa Good Shepherd offers all the
amenities you expect for comfortable living, while our Christian philosophy of
care provides a setting for fostering lasting friendship and a sense of
belonging.
In our one- or two-bedroom single-level apartments we offer convenient and
important services which help you maintain your independence. The Good Samaritan
Society- Mesa Good Shepherd provides each apartment with an emergency call
system and general housekeeping (every two weeks). You can rest assured that our
staff will do the maintenance and landscaping. All utilities are also included,
with the exception of cable television and phone services. If you have a
vehicle, covered parking is included. However, if you don’t have a vehicle or
don’t feel like driving, we make available scheduled transportation for
shopping, entertainment, errands, and medical/dental appointments within a
15-mile radius at no extra charge.
For your enjoyment and pleasure, we offer activities, wellness programming,
spiritual ministries, and optional clubhouse dining. Come enjoy our beautiful
chapel, clubhouse, wellness center, jacuzzi, pool, and putting green. The
clubhouse is available to schedule private parties or events.
For those who need additional care and services we provide "Valet Services,"
additional non-medical services available at an option. Our campus health
manager is available for health and safety consultation and valet scheduling. In
addition, our senior living apartment residents are also afforded priority
admission into our assisted living center or Healthcare and Rehabilitation
Center, depending on appropriate insurance or payment source and space
available.
