Caring vs. Providing Care
Our Assisted Living Service Program is focused on nurturing residents to help them reach their maximum potential. Our staff seeks to foster independence rather than dependence, and to be sensitive to the individuality, privacy and freedom of choice to which our residents are entitled. In short, care is something we do, not simply a service we provide.

We meet the changing needs of our residents by offering a fee structure that addresses the degree of need and frequency of interaction. The resident's monthly fee may increase or decrease as their needs change. Our trained staff documents services provided, and notes trends that may demonstrate a higher level of need.

Personal Pathways Activity Program
The journey along a pathway is often more important than the destination, especially in dementia care. Personal Pathways is Horizon Bay' program for the Garden House that focuses on meaningful activities. Our goal is to help maintain independence and quality of life during the progression of Alzheimer's and related dementias. Personal Pathways offers activity choices that allow each resident to spend time in ways that can be meaningful and enjoyable to them. Each resident has a Personal Life History completed upon move-in to let us know of individual activities that may be of interest to them. Specially trained caregivers incorporate meaningful activities into daily life in the Garden House. Caregivers are encouraged to facilitate creative activities respecting the uniqueness of each community and individual. The Personal Pathways component allows individual differences in locale, culture, needs and interests to be accommodated. Family participation is encouraged.
Family Support
It is important to help each resident continue to be involved with their family. Family members are encouraged to participate in the life of their loved one at Horizon Bay. We want you to take advantage of support groups and participate in service planning meetings to increase your understanding of dementia, and to help us shape how it impacts your loved one and family.
