Welcome to Evergreen Valley Adult Day Care! The Adult Day Services offer the opportunity for participants to remain at home and give families the freedom to work and enjoy their lives while maintaining a close relationship and responsibility for their loved one.
Our Adult Day Services is a medical model that provides supportive services for families or spouses that desire to keep their loves ones at home. The center offers a wide variety of health and social assistance. A registered nurse provides health assessment and treatment as well as assistance with medications.
Program Assistants provide personal care and structured Calendar of Events. Physical therapy, nutritional and social work counseling are also available. Evergreen Valley will furnish transportation to the Center and medical appointments as well as provide a hot noon meal.
